Other Quality Initiatives


CCHN is one of five Regional Coordinating Centers for the Safety Net Medical Home Initiative (SNMHI), a four-year national demonstration project to accelerate medical home transformation, and reach high levels of quality, efficiency, and patient experience. Initiative partners include The Colorado Health Foundation, The Commonwealth Fund, Qualis Health, and the MacColl Institute for Healthcare Innovation.

Thirteen Colorado safety net clinics were selected to receive technical assistance on practice redesign topics such as enhanced access, patient centeredness, and care coordination. The initiative calls for partnerships between safety net providers and community stakeholders to work towards a new model of primary health care delivery that has shown promise as a way to provide high-quality, cost effective health care to low-income and minority patients while simultaneously reducing racial and ethnic health disparities.

National Accreditation Programs
Many Colorado CHCs participate in national accreditation and recognition programs, which designate safety net clinics as meeting standards for the delivery of high quality and efficient health care services to patients. CCHN provides technical assistance and coordinates trainings for CHCs who are (or would like to become) accredited by the following programs:
