Other Quality Initiatives


In addition, CCHN’s Quality Initiatives Division provides group and customized training and technical assistance, practice coaching, and data analysis on a variety of quality improvement activities, including:

  • Developing a quality improvement plan
  • Training staff on various quality improvement methodologies
  • Providing on-site assessments of clinic functionality and areas for improvement
  • Establishing quality improvement committees
  • Implementing patient or staff satisfaction surveys
  • Data analysis for quality improvement

Colorado Community Health Network’s Institute for Healthcare Improvement® Travel Support Program
Kaiser Permanente Colorado on behalf of its Community Benefit Programs awarded Colorado Community Health Network (CCHN) with a grant to assist Community Health Centers (CHCs) employees with travel expenses to attend the Institute for Healthcare Improvement® conferences.  

Meaningful Use
Under American Recovery and Reinvestment Act of 2009, health care providers that demonstrate Meaningful Use of Electronic Health Information Technology are eligible for financial incentives.  To qualify, clinicians must use Electronic Medical Record Systems to improve the quality of patient care, operational efficiency, and chronic disease management. CCHN in collaboration with Colorado Community Managed Care Network (CCMCN) provides Colorado’s CHCs with technical assistance to meet and exceed Meaningful Use standards.

Accountable Care Organizations
The Colorado Accountable Care Collaborative (ACC) provides opportunities for Community Health Centers and other Safety Net Providers to leverage experience as Patient Centered Medical Homes as both providers in the ACC program and organizational partners in the Regional Care Collaborative Organizations. The goal of the ACC is to improve quality and reduce costs in Medicaid.
