CCHN partners with CHCs to promote safe and timely vaccination of children. The immunization initiative includes implementing and evaluating quality improvement activities in immunization delivery through information sharing, data collection, and analysis in order to improve immunization rates. The immunization initiative began in 1995 with CCHN’s involvement in the Together For Tots Program. Currently, CCHN and CHCs are partnering in a health information technology quality improvement project.
- CCHN provides technical assistance for quality improvement in immunization delivery, and analysis of data compiled from electronic medical records (EMRs), the Colorado Immunization Information System (CIIS), and the Colorado Associated Community Health Information Exchange (CACHIE).
Through the combined efforts of CHCs and CCHN, Colorado CHCs improve immunization rates among young children. Many clinic sites have surpassed the national benchmark rate of greater than 90 percent for one-year-olds and greater than 80 percent for two-year-olds. CCHN’s goal is to work together with CHCs to continue to improve immunization rates benefiting Colorado’s children.
