- CKF Brief: Innovative School-Based Outreach and Enrollment Tactics for Medicaid and CHP+ - Fall 2009
CKF produced a brief highlighting the outreach and enrollment work done by three school-based pilot projects that make it easier for families to enroll their children into Medicaid and Child Health Plan Plus (CHP+) through schools. These three pilot sites find low-income, uninsured children by sharing necessary information as families apply for free- or reduced-price lunches and are able to determine eligibility for Medicaid and CHP+ on site. - CKF Report: Case Management Report - August 2008
CKF staff and Coalition members developed recommendations regarding the additional $1.4 million in HCPF's budget dedicated to Medicaid and CHP+ outreach. A Case Management Approach to Medicaid and CHP+ Enrollment- Recommendations for Colorado's Investment in Medicaid and CHP+ Outreach outlines an approach that supports families and the sites that assist them with Medicaid and CHP+ enrollment. This paper is based on a comprehensive definition of outreach that includes informing families about the programs, providing application assistance, promoting access to care, and ensuring renewal. In addition, the paper makes the case that sites need adequate training and financial support in order to provide enrollment assistance.
- Children EBNE in Medicaid and CHP+ - March 2011 Colorado Health Institute (CHI) publishes annual estimates of Colorado children eligible but not enrolled (EBNE) in Medicaid and CHP+ by county to help state and local policymakers understand enrollment patterns in their communities. Resources to Help Community Organizations Complete Applications
Listed below are fact sheets, forms, and other materials developed by Community Based Organizations that might be useful to other sites.
- Self Employment Log - An example for families of what they can develop to maintain their own information related to self-employment income and expenses. Self- employment income is self-declared for Medicaid and CHP+ applicants so families do not need to submit this form with their application. It's meant to be a tool to help families get organized.
- Sample Stamp for Verifying DRA Documents - First time Medicaid applicants and renewing CHP+ clients must either submit original documents (NOT recommended) or have their documents verified by a document verification site. This stamp serves as a model for sites that want to develop their own stamp to streamline the process of verifying documents.
- Case Summary Form - Provides a place to consolidate all case information including demographics, PE and Physician information, referrals, and income.
