Colorado Community Health Network (CCHN) – Comprehensive Needs Assessments (CNAs) for 10 Colorado counties. (2009): CCHN contracted with Ray & Associates to conduct CNAs for 10 Colorado counties as part of a Planning Grant it received from HRSA in 2008. County Indicators
The County Indicators are available through the Colorado Health Institute.  The County Indicators are a report made available through the joint efforts of several entities and provide a snapshot of county level data including county demographics, health indicators and access indicators.  County reports are available, click here.

Uniform Data System
