Data

Colorado Community Health Network (CCHN) – Comprehensive Needs Assessments (CNAs) for 10 Colorado counties. (2009): CCHN contracted with Ray & Associates to conduct CNAs for 10 Colorado counties as part of a Planning Grant it received from HRSA in 2008. Colorado Health Information Dataset
The Colorado Health Information Dataset is an interactive data system from the Colorado Department of Public Health and Environment including birth, death, population, behavioral risk factors, cancer, pregnancy, injury, and tobacco statistics.  To access the dataset click here.

Colorado Health Institute Data Resources
The Colorado Health Institutes maintains up-to-date information on what data is available, where data is available, and at what level data is available.  For more information click here.

County Indicators
The County Indicators are available through the Colorado Health Institute.  The County Indicators are a report made available through the joint efforts of several entities and provide a snapshot of county level data including county demographics, health indicators and access indicators.  County reports are available, click here.
 
Health Resources and Services Administration Geospatial Warehouse
The Geospatial Warehouse offers a number of resources including mapping, demographic data, and Federal grantee information and profiles.  For more information click here.

Uniform Data System
The Health Resources and Services Administration (HRSA) Bureau of Primary Health Care (BPHC) collects data annually from the existing CHCs on information appropriate to monitoring and evaluating CHCs, including user characteristics, financial information, and select clinical information.  To request a copy of the National or Statewide aggregate report click here.
