CCHN’s mission is to increase access to high quality health care for people in need in Colorado. CCHN's mission is accomplished by numerous divisions within the organization, including:
Policy Division
The Policy Division advocates legislative and regulatory policies that will increase access to health care for the medically underserved, including Medicaid recipients, Children Health Plan Plus (CHP+) recipients, and those who are uninsured or underinsured. The division:
1. Promotes policies that will support CHCs in their role as the largest, proven primary health care system in the state;
2. Coordinates a statewide grassroots network of citizen activists;
3. Includes Colorado Covering Kids & Families, a coalition-based project that works to ensure that all eligible children and adults are enrolled in public health insurance programs;
4. Provides CCHN news and public relations.
Division members include: Polly Anderson, Stacey Moody, Maureen Maxwell, Katie Jacobson, and Stephanie Arenales.
Health Center Operations Division
The Health Center Operations Division (HCOPS) focuses on growing, strengthening, and sustaining CHCs in Colorado. The HCOPS Division provides technical assistance, training, data analysis, and other resources to CHCs and other partners in the areas of:
1. Health Center Operational and Financial Systems;
2. Health Center Workforce Development Activities;
3. Health Center Community Development Activities; and
4. Health Center Emergency Preparedness
The Health Center Operations Division maintains effective relationships with partners to promote and support CHCs in Colorado.
Division members include: Belinda Prawdzik, Nancy Gilbert, Suzanne Smith and Tanah Wagenseller.
Clinical Quality Division
In the fall of 2007 all of CCHN's clinical programs merged into one division. This meant that the Health Disparities Collaborative Division joined the Clinical Quality Division!
The Clinical Quality Division (CQD) positively impacts the quality of clinical services at CHCs by providing technical assistance, training, data collection & analysis, and other resources to Colorado's CHCs and other partners in the areas of:
1. Cancer Prevention & Detection including the Healthy Woman Outreach Project, the Women's Wellness Connection and the Colorado Colorectal Screening Program;
2. Oral Health including the Colorado Oral Health Network (COHN);
3. An Immunization Initiative and Vaccines for Children;
4. Prenatal & Family Planning. The Health Disparities Collaborative (HDC) provides technical assistance, training, data analysis and other resources to Community Health Centers (CHCs) in Colorado in the areas of:
1. Generating and documenting improved health outcomes for underserved populations;
2.Transforming clinical practice through models of care, improvement and learning;
3. Developing infrastructure, expertise and multi-disciplinary leadership to support and drive improved health status;
4.
Building strategic partnerships
The HDC works with national partners to promote the quality of care to the CHCs that we support in Colorado. The Clinical Quality Division assists CHCs in providing high quality patient care. Division members include: Molly O'Fallon, Jessica Sanchez, Brenda Lantz, Janie Martinez, Brandi Raiford, Paula Lueras and Paula Davis.
